Band 7 Social Worker – Channel Island

Key Responsibilities 

• Manage a caseload of complex cases, ensuring high-quality support and outcomes for individuals and families
• To be the lead Social Worker for the Rapid Response and Reablement team within the Integrated Adult Community Care Service, acting as line manager to social work staff where required.
• To form relationships with service users to formulate an accurate assessment of complex psychosocial needs of service users, their families and service providers and assess for services in line with local policy.
• To contribute towards a holistic assessment within the multi-professional specialist team, recognising that diversity, values and human rights may influence the decisions of people who use the service.
• To provide specialist social work advice within the Adult Community Health & Social Care Team.
