Healthcare facility in West Midlands is seeking two experienced Lead Complex Case Managers to join our team. This is an exciting opportunity to make an impact on patient care across the region.
Key Details:
- Role: Lead Complex Case Manager (Band 8a)
- Location: Hybrid Working (Home/Patient Facing)
- Hours: Monday to Friday, 9am – 5pm (37.5 hours per week)
- Contract: Initial 6-month term
- Patient Interaction: Yes, occasional visits across Birmingham and Solihull
Responsibilities:
- Manage complex patient cases, ensuring high-quality care and support.
- Coordinate with healthcare professionals and service providers.
- Conduct patient visits as necessary.
- Develop and implement care plans tailored to individual patient needs.
- Provide leadership and guidance to case management teams.
Requirements:
- Proven experience in complex case management within the NHS or similar healthcare settings.
- Strong leadership skills and the ability to manage and support teams.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Registered Nurse or relevant healthcare qualification.
Essential
- Professional Qualification and current registration with relevant professional body i.e. NMC/HCPC (Nurse/SW/OT/AMHP)
- Educated to degree level or equivalent level of experience working in a specialist area
Experience

- Previous experience of undertaking Continuing Care Assessments
- Experience of negotiating and liaising with multi-disciplinary team.
Knowledge
Essential
- Knowledge of MCA DOLS LPS
- Good understanding of Safeguarding issues and policy framework
